Thank you for choosing UUANJ for your Urology Specialist. We want to make sure your visit to our practice as pleasant as possible. Therefore, we request that you follow these guidelines when visiting our practice.
If you are interested in becoming a new patient at UUANJ, please call us at (609) 581-5900 , (732) 286-6644, or (732) 928-5300 to schedule an appointment.
Please download and complete the appropriate new patient packet by clicking here. These forms are fillable online for your convenience. If you are not able to complete your patient registration prior to your visit, you will be given the same document upon your arrival, but we ask that you arrive 30 minutes before your scheduled appointment time to fill out the paperwork.
Our office is fully electronic, utilizing an electronic health record, so it is important that you fill out your patient history and review of systems with as much detail as possible. Upon completion of your registration, our staff will attach your registration paperwork directly into your electronic chart. Please bring your insurance card and driver’s license with you to your appointment. These will also be scanned into our system. We will take a picture of you to place in your electronic record when you check-in, and collect your co-payment (if applicable).
If your insurance policy requires that you obtain a referral for specialist care, please obtain one prior to your appointment. If you have questions about this, please do not hesitate to contact our office.
